Five dead after crash in San Juan County

MONTICELLO, Utah (ABC 4 Utah) -- Five people are dead after a head-on collision on State Route 191 Sunday afternoon.
MONTICELLO, Utah (ABC 4 Utah) -- Five people are dead after a head-on collision on State Route 191 Sunday afternoon.

Utah Highway Patrol said a white van with five people inside attempted to pass on the highway. The van collided with a car traveling in the opposite lane. The collision killed three of the five people inside of the van as well as both the driver and lone passenger inside the second vehicle.

UHP is still investigating the crash.

Monday officials released the identities of those in the crash:

Occupants of the 2010 Chrysler / Town and Country:

Fatal victims-
Driver: Samantha Blueeyes, 23, of Salt Lake City
Alfreda A Bowman, 28, of Salt Lake City
Michael A. Blueeyes, 22, of Salt Lake City
Esmerelda Velasquez, 11, of Salt Lake City

Single survivor from the Chrysler Town and Country:
Travis L Howland, 24, of Salt Lake City. Howland is still in very critical condition.

Occupants of the 2007 Chrysler / Crossfire:
Cody R Farrabee, 22, of Queen Creek, Az
Rheana Hazel, 23, Las Vegas, NV
