The report says the Suns and Hornacek have finalized a 3-year contract. The deal is expected to be officially announced as early as Sunday night.
Hornacek played the first six seasons of his career with the Suns from 1986-1992. He was hired by the Jazz in 2007 as a part-time shooting coach, and has been a full-time assistant with Utah under Tyrone Corbin since 2011.
Hornacek, 50, played with the Jazz from 1994-2000, helping the Jazz reach the NBA Finals in 1997 and 1998. His #14 jersey was retired and is hanging in the rafters at Energy Solutions Arena.
Hornacek will have his work cut out for him in Phoenix. The Suns just completed its second worst season in franchise history with a record of 25-57, finishing dead last in the Western Conference.
Hornacek also interviewed to become the new head coach of the Charlotte Bobcats and Philadelphia 76ers.
